Understanding Skilled vs. Residential Facilities

The Always Home Residential Assisted Living Difference

Choosing the right home for yourself or a loved one is about more than a label—it’s about care, safety, trust, and experience. Families often ask how medical needs are handled in skilled vs residential settings. At Always Home Residential Assisted Living, we believe in full transparency and proactive care.

What Skilled Means in North Carolina

In North Carolina, a skilled facility is approved by the state to provide regulated personal care and certain health-related services. An residential assisted living setting is not required by law to provide those skilled medical services or to operate under the same regulatory structure.

Always Home is a Residential Assisted Living Home.
However, our standards of care are intentionally higher than what is legally required for an residenital setting.

How Always Home Operates—By Choice, Not Requirement

Although we are not required by law to operate as a licensed facility, Always Home voluntarily operates much like one, based on the owner’s extensive background in nursing and healthcare.

What this means for families:

✔ Nursing-informed health monitoring
Residents are observed daily for changes in condition such as confusion, falls, appetite changes, skin concerns, or signs of illness. Changes are communicated promptly to families and appropriate providers.

✔ Structured care planning
While not required by law, Always Home develops individualized care plans for each resident to promote consistency, dignity, and safety.

✔ Emergency preparedness & response
Staff follow established emergency response protocols for situations such as falls, breathing concerns, or sudden health changes, including timely notification of emergency services and families.

✔ Infection control practices
Always Home follows healthcare-based infection control principles, including sanitation, hygiene, and illness prevention measures—even though state oversight is not required.

✔ Internal policies & procedures
As a company policy, Always Home maintains written policies and procedures related to:

  • Health observation

  • Emergency response

  • Infection prevention

  • Documentation and communication

These policies exist because we believe residents deserve structure, consistency, and accountability, not because we are required to have them.

Important Medical Services We Do Not Provide (By Law)

To remain compliant with North Carolina law, Always Home does not provide skilled or licensed medical services, including:

  • Medication administration or medication management
  • Wound care or dressing changes
  • Skilled nursing services

How these needs are addressed:

  • Licensed home health agencies
  • Hospice providers
  • Physicians and other medical professionals

We actively coordinate with contracted home health and hospice providers to ensure residents receive the medical services they need—while remaining within our scope of practice.
